  • What does a weekly record of eating – listing every meal – look like?
  • Do you eat sugar?
  • Do you drink alcohol?
  • Do you smoke?
  • Do you eat fresh vegetables daily?
  • Do you eat packet meals?
  • Do you eat fresh meat, poultry or fish daily?
  • Do you drink carbonated drinks?
  • Do you eat desserts?
